TITLE : Early Start Infant Services Coordinator  

 

BIWEEKLY SALARY : $2,058.98 - $2,502.70

STARTING DATE : Position Open Until Filled

POSITION DESCRIPTION:

Under the supervision of the Program Manager - Early Start Program, the Infant Services Coordinator is the first agency representative to families who have infants eligible for services under Part C. Is responsible for assessment of these infants, educating parents regarding developmental strategies, arranging for appropriate service intervention, making appropriate case dispositions, and liaison activities with referring professionals. Develops and implements Individual Family Service Plan (IFSP). Performs case management activities and community liaison tasks related to these cases. Completes the intake process. Has no supervisory responsibilities.

MINIMUM QUALIFICATIONS/CRITICAL CLASS REQUIREMENTS:

EDUCATION : BSN in Nursing and possession of a certificate in public health nursing from an accredited school, or a Master's Degree in a related field from an accredited school.

EXPERIENCE : Prior experience in high-risk infant follow-up is desired.

OTHER: Full use of an automobile, possession of a valid driver's license and automobile liability insurance for the minimum amount prescribed by law, or ability to provide for independent transportation. Must have and maintain a safe driving record.

WORKING CONDITIONS AND PHYSICAL REQUIREMENTS : The majority of these job duties are performed in the field and in the office. The ability to drive an automobile is absolutely required on a daily basis. The ability to sit, stand or walk for moderate distances on uneven or difficult terrain and the ability to enter inaccessible facilities is also required on a daily basis. Daily, 4-6 hours of reading, writing and phone work are required. Good oral and written communication skills are required as is the ability to utilize office equipment requiring manual dexterity.
