How Do I Reach Or Contact My Consumer Service Coordinator?

After you or your family member is determined eligible for services, you will receive a letter notifying you of the name of your assigned Consumer Service Coordinator. Shortly thereafter, your Consumer Service Coordinator will contact you to arrange a convenient time for your first meeting. This meeting can take place at your home, our main office in San Bernardino, or at one of our conveniently located auxiliary offices in your local community. After this initial meeting, you will be able to contact your Consumer Service Coordinator by telephone, or by scheduling an appointment for another visit at your home, at our office or at an auxiliary office in your area.

Our office has a telephone voice mail system, so that you can leave a voice message for your Consumer Service Coordinator even if they are not in the office. Your Consumer Service Coordinator picks up voice messages regularly, and will usually return your call by the end of the next working day. When leaving a voice mail message, it is helpful to provide your name, phone number and a brief message about why you are calling. If you are a parent and your child has a different surname from your own, please include your child’s name when leaving a message. If you feel that your telephone calls are not being returned in a timely manner, you can ask the telephone operator to connect you with the Program Manager for your assigned Consumer Service Coordinator.
