TITLE : Case Management Trainee
BIWEEKLY SALARY : $1,147.88
STARTING DATE : Position Open Until Filled
POSITION DESCRIPTION : Under the direct supervision of the Program Manager, participates in a year long training position and is responsible for the direct case management services offered to consumers by the program; serve consumers in a small caseload (25-40) for at least the first six months. After six months, Case Management Trainee is responsible for a regular caseload, and sees to it that entire case management process with concomitant authorizations and paperwork are completed; has advocacy responsibilities. Perform related work as assigned. Does not have supervisory responsibilities.
MINIMUM QUALIFICATIONS/CRITICAL CLASS REQUIREMENTS :
EDUCATION : BA in social work, psychology, or a related field from an accredited school.
AND
May be required to be bilingual.
OTHER : Full use of an automobile, possession of a valid driver's license and automobile liability insurance for the minimum amount prescribed by law or ability to provide for independent transportation. Must have and maintain a safe driving record.
WORKING CONDITIONS AND PHYSICAL REQUIREMENTS : The majority of these job duties are performed in the field and in the office. The ability to drive an automobile is absolutely required on a daily basis. The ability to sit, stand or walk for moderate distances on uneven or difficult terrain and the ability to enter inaccessible facilities is also required on a daily basis. Daily, 4-6 hours of reading, writing, and phone work are required. Good oral and written communication skills are required as is the ability to utilize office equipment requiring manual dexterity.
