Every year, the Department of Developmental Services (DDS) sends a Family Outcomes Survey (FOS) to a randomized group of thousands of families who have participated and are participating in the Early Start Program. We are excited to share that the Department has sent the annual survey to families.

If your family is selected to receive one of these surveys, we strongly encourage you to complete the survey on or before November 30, 2024. While the survey asks for some information about your family, including your child’s birthdate, please know that the information is coded in an anonymous format and is only used in aggregate data.
